Consumer Rights & Important Information
In accordance with Texas House Bill 4224 (89th Regular Session) and Section 181.105 of the Texas Health and Safety Code, consumers have the right to access their health care records and to file complaints regarding health care services.

Requesting Your Health Care Records
You may request copies of your health care records directly from the Anchor Counseling, PLLC office. Requests must be submitted in writing. Please contact Kaci Bryant, MA, LPC for the appropriate authorization form and instructions.
